8th Annual Teen to Adult Healthcare Transition Summit

This will be a hybrid model conference, focusing on teen-to-adult healthcare transition. Topics will include aging out of public services, accessing services at colleges, and adaptive care plans. Lastly, patient awards and videos will be presented, where we will honor many teens and young adults who have gone above and beyond in caring for their health.

1. Discuss real life healthcare transition experience including tools available at public and institutional levels
2. Identify resources for healthcare transition and the best practices for their usage

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Intermountain Health, Intermountain Children's Health, the Department of Pediatrics at the University of Utah School of Medicine, and the Utah Department of Health and Human Services. Intermountain Health is accredited by the ACCME to provide continuing medical education for physicians.

Nursing Accreditation

Intermountain Health is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation. This live activity offers a maximum of 4.25 nursing contact hours. This activity is jointly provided by Intermountain Health, Intermountain Children's Health, and the Department of Pediatrics at the University of Utah School of Medicine. Successful completion is attending no less than 1 full session and claiming credit only for sessions attended. Credit is awarded after submission of post-conference survey.
