This activity is designed to help providers understand the foundational principles of risk adjustment and how their clinical documentation directly influences patient risk scores, reimbursement, and care outcomes. Through real-world examples and practical strategies, participants will learn how to optimize documentation workflows without increasing administrative burden - ensuring clinical complexity is accurately captured and reflected.

1. Analyze risk adjustment and verbalize how accurate documentation and coding influence risk scores, quality metrics, and reimbursement.
2. Apply MEAT (Monitor, Evaluate, Assess, Treat) criteria to support accurate and complete documentation of chronic conditions.
3. Apply risk adjustment strategies to accurately reflect patient complexity, supporting improved care planning, population health management, and value-based performance.
