7th Annual Teen to Adult Healthcare Transition Summit

This will be a hybrid model conference, focusing on teen-to-adult healthcare transition. A panel of rural area providers will discuss their challenges/successes, and the Disability Law Center will present on key healthcare related legislative changes. We are excited to welcome the Utah Parent Center Youth Advisory Board, who will be speaking on self-advocacy. Lastly, patient awards and videos will be presented, where we will honor many teens and young adults who have gone above and beyond in caring for their health.

1. Discuss real life healthcare transition experience of clinicians serving in rural population.
2. Discuss and examine new legislative changes that might impact providers, youth, and families in healthcare.
3. Present teen-produced videos showcasing a youth's healthcare transition journey and successes along the way.

Both virtual and in person registration options available.

Registration fee $15.00 per person.

*Community youth and family members: no registration fee.

Refunds:

Cancellation and Reimbursement Policy: Full (100%) registration reimbursement for cancellation more than 14 days prior to the beginning of the activity. Half (50%) registration reimbursement for cancellation between four (4) and fourteen (14) days prior to the beginning of the activity. No reimbursement for cancellation three (3) days or less prior to the beginning of the activity.

Exceptions:

Exceptions may be allowed only under extenuating circumstances and with approval from the IPCE Director, IPCE Medical Director, and VP for IPCE.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Intermountain Health, Intermountain Children's Health, the Department of Pediatrics at the University of Utah School of Medicine, Utah Department of Health and Human Services, Utah Parents Center, and the Utah Disability Law Center. Intermountain Health is accredited by the ACCME to provide continuing medical education for physicians.
