This is a conference designed to discuss teen to adult healthcare transition related issues. Presenters and small group discussions give real life experience, expertise and recommendations for this high risk population.


Objectives
  1. Analyze the aspects of Teen to Adult Healthcare Transition framework.
  2. Determine how to apply the framework working with teens and young adults with special healthcare needs.

Registration

There is no fee to attend this summit, however registration is required.

 *This is a hybrid conference, both in-person and virtual options are available. Please select your preference when registering. In-person space is limited. Those who choose to attend virtually will be sent a link to join us online.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Intermountain Healthcare, Primary Children's Hospital, the Department of Pediatrics at the University of Utah School of Medicine and the Utah Department of Health & Human Services.
