1. Participants will be able to understand Intermountain’s vision for proactive care and its role in transforming healthcare delivery.
2. Participants will have the tools they need to explain how patient segmentation and team-based care support a proactive approach across acute and ambulatory settings.
3. Participants will be able to identify opportunities to advance proactive care within their own role and area of responsibility.
