Overview

The primary focus of this 2-hour class is an in-depth review of hemodynamic and oxygenation parameters. Specific values to be discussed include:

Central Venous Pressure, Pulmonary Artery Pressure, Pulmonary Vascular Resistance, Pulmonary Artery Wedge Pressure, Left Ventricle End Diastolic Pressure, Systemic Vascular Resistance, Cardiac Output, Cardiac Index, Stroke Volume, Stroke Volume Index, and Ejection Fraction. Discussion of oxygenation parameters includes an analysis of Arterial, Mixed Venous and Central Venous Blood Gases. The class also includes a discussion of basic maintenance and set-up of a pulmonary artery catheter and the Edwards HemoSphere for continuous cardiac output monitoring, as well as a live demonstration of both. Participants should have a sound working knowledge of the anatomy and physiology of the heart and lungs and be proficient with understanding and interpreting basic vital signs.

Objectives
  1. State the indications for use of a PAC.
  2. State which hemodynamic values represent preload, what that concept implies, and what interventions we can take to affect it.
  3. State which hemodynamic values represent afterload, what that concept implies, and what interventions we can take to affect it.
  4. State which hemodynamic values represent contractility, what that concept implies, and what interventions we can take to affect it.
  5. State the indications, normal ranges/doses, and side effects of commonly used vasoactive and inotropic drips.
  6. State the normal values for a PaO2, SvO2 and lactate and what abnormal results indicate.
  7. Demonstrate basic nursing care for a patient with a PAC (how to lock, measure insertion depth, interrogate all lumens, secure in place, and hook up to the Phillip’s Monitor and Edward’s HemoSphere).
  8. Demonstrate basic set-up of the Edward’s HemoSphere monitor
