6th Annual Teen to Adult Healthcare Transition Summit

Hybrid model conference (in-person & online) focusing on teen to adult healthcare transition. Our community partner, the Utah Parent Center, along with pediatric and adult providers, youth, and families will discuss crucial healthcare transition topics.

1. Examine and review aspects of guardianship related to teen to adult healthcare transition structure, processes, and resources.
2. Discuss real life experiences of youth, young adults and their families as they transition to adult healthcare.
3. Presentation of films and awards showcasing youth’s healthcare transition journey and successes.

There is no fee to attend this summit, however, registration is required.

Both Virtual and in-person attendance are available.
