This conference will educate healthcare providers within Enterprise Care Management. The focus this year will be on Proactive Care across the continuum, and self-care for caregivers and others.

ANCC Learning Outcomes
  1. Caregivers will self-report an increased knowledge and comprehension regarding best practices in care management across the continuum of care, and how this aligns in all 3 regions (Desert, Canyons, Peaks.)
  2. Caregivers will self-report an increase in knowledge, proficiency, and reasoning behind best practices in self-care.
  3. Caregivers will self-report an increase in knowledge and skill to implement best practices in change management and to cultivate personal resilience.

Accreditation

ACCREDITATION

Intermountain Health is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center’s Commission on Accreditation. This activity is jointly provided by Intermountain Health and Proactive Care. This live activity offers a maximum of 3.75* nursing contact hours. Successful completion is attending no less than 1 full session and claiming credit only for sessions attended.

*Credit hours are subject to change and are based on actual learning hours and at the discretion of IPCE.
